For over 60s - no upper age limit, no health questionnaire
Unlimited general practitioner (GP) & specialist visits + home & senior care
Outpatient only - no hospital cover, not visa-compliant
Sanitas Unico is private health insurance for people aged 60 and over in Spain, with no upper age limit to join or stay insured and no health questionnaire. Premiums start at €48,10 a month. It covers doctors, specialists, scans and home services, but not hospitalisation or surgery, and is not accepted for a visa.
Sanitas Unico is a private health insurance plan built specifically for people aged 60 and over living in Spain. Unlike most policies that have an upper age limit, Unico accepts new members from age 60 with no upper age limit, no health questionnaire, and no pre-existing condition exclusions.
In Spanish the plan is written Sanitas Único, with an accent on the U. It is the same plan, sold across Spain by Sanitas, part of the Bupa group, and this page describes it in English.
The plan covers the day-to-day healthcare needs that matter most as you get older: unlimited primary care and specialist consultations, the most common diagnostic tests (including ultrasounds, magnetic resonance imaging (MRI) and computerised tomography (CT) scans), simple therapies like physiotherapy, basic dental cover, and a full suite of digital and home services through Sanitas Blua and Sanitas Mayores.
Unico does not cover hospitalisation, surgery, or complex therapeutic methods - these are handled separately. If you need hospital cover, see Mas Salud or Top Quantum on our compare plans page.
Diagnostics Simple tests + ultrasounds, MRIs and CT scans
Sanitas Mayores Senior care plan, home services, dependency advice
Digital care Blua video consultations (24h emergency line), health programmes, Take Care of Your Mind
Important: Sanitas Unico is an outpatient and home-care plan only. It does NOT cover hospitalisation or surgery, and it is NOT suitable for residency or non-lucrative visa applications. If you need hospital cover or a visa-compliant plan and you're over 60, consider Mas Salud or Top Quantum.
Why this plan exists: Most private health insurance becomes expensive or completely unavailable after age 65-70 due to underwriting. Sanitas Unico removes that barrier - designed from the ground up for over-60s, with senior-specific services (Sanitas Mayores, home care, dependency advice) that other plans don't offer.

*High-frequency services, where a single co-payment of €20 covers the full course. They are 10 physiotherapy sessions, 15 days of aerosol therapy, 15 ventilation therapy sessions and 30 days of oxygen therapy. The same applies to 30 days of bilevel positive airway pressure (BPAP) or continuous positive airway pressure (CPAP) therapy.
Quick example: If you make 8 GP/specialist visits (the first 6 are free, the next 2 are €5 each), 1 emergency visit (€8), and 2 simple diagnostic tests (€12 + €12), your total co-payments for the year are €42. Have no more than 6 consultations in the year, and no tests, emergencies or other services with their own co-payment, and you pay €0 in co-payments - only your monthly premium.
Note on video consultations: Video consultations with specialists count as in-person consultations for the purposes of the co-payment schedule above (that is, they go into the 0-6 / 7-15 / 16+ tier counter).
How much does Sanitas Unico cost?
Premiums are charged per insured person, per month. There is no annual-payment discount, and no extra premium loading by individual age within each band - premiums are fixed at the band level.

*New-customer pricing with a 20% discount on the base premium, available for current promotional periods set by Sanitas. A 0.15% surcharge from the Insurance Compensation Consortium is added. Prices may vary by region (zona geografica). Confirm the latest premium when you request a quote.

What is Sanitas Unico?
Sanitas Unico is private health insurance for people aged 60 and over in Spain, insured by Sanitas, part of the Bupa group. It has no upper age limit to join or to stay insured, no health questionnaire and no exclusions for pre-existing conditions. It is an outpatient plan with co-payments: it covers consultations, tests, simple therapies, emergency care, basic dental cover, home services and Sanitas Mayores senior care, but not hospitalisation or surgery.
What does Sanitas Unico cover?
Sanitas Unico covers unlimited in-person GP and specialist consultations, simple diagnostic tests, ultrasound, CT and MRI scans, and simple therapies such as physiotherapy. It also includes up to 15 psychology sessions a year with prior authorisation, basic dental cover, emergency care in Spain, home services and the Sanitas Mayores services. It does not cover hospitalisation, surgery of any kind, complex therapies, ambulance services or care outside Spain. Consultations, tests and therapies are given in the Sanitas medical network, which you can search in our medical directory.
Is there really no upper age limit on Sanitas Unico?
Sanitas Unico has no upper age limit: new members join from age 60 with no maximum entry age, so you can take out Unico at 70, 80, or even 90. There is also no maximum age to remain insured: once you're a member, Sanitas does not cancel your policy because of age. This is the key reason the plan exists.
Which Sanitas plan is best for seniors over 60?
Sanitas Unico is the Sanitas plan built for seniors: it is sold from age 60, has no upper age limit, needs no health questionnaire and includes the Sanitas Mayores services. If you need hospital cover, surgery or a policy for a visa or residency, Sanitas Mas Salud and Sanitas Top Quantum accept new members up to age 75, subject to underwriting. Read our guide to healthy ageing in Spain.
Is Sanitas Unico a good plan?
Sanitas Unico is a good plan for people aged 60 and over who live in Spain and want private access to doctors without a health questionnaire. It covers GP and specialist visits, tests including MRI and CT scans, emergency care and home services, and there is no age at which it closes to new members. If you need hospital cover, surgery or a policy for a visa, compare Sanitas Mas Salud and Sanitas Top Quantum in the plan comparison above. Read our review of Sanitas health insurance in Spain.
Do I need to fill in a health questionnaire for Sanitas Unico?
No. Sanitas Unico does not require a health questionnaire and has no underwriting based on your medical history. Pre-existing conditions are not excluded.
Can I use Sanitas Unico for a residency or non-lucrative visa application?

Apart from blood tests at home, these services apply after more than 48 hours in hospital, or when a medical certificate shows you are housebound for more than 5 days. See the full home services table for annual allowances.
What is Sanitas Mayores and is it included?

Can I use Sanitas Unico outside Spain?
No, Sanitas Unico does not cover you outside of Spain. For worldwide cover see our international plans.
Are there waiting periods on Sanitas Unico?
Almost none. The only services with a waiting period are MRI scans and CT scans (6 months), and only for new customers who don't already have private health insurance. Ultrasounds and all other services have no waiting period. If you are transferring from another insurer's full-cover plan, the MRI and CT waiting period is also waived.
How much does Sanitas Unico cost?
Sanitas Unico premiums depend on your age band: €48,10 / month at 60-64, €53,40 / month at 65-74, €61,20 / month at 75-85, and €72,50 / month for over 85s. These are new-customer prices with a 20% discount on the base premium. There is no extra premium loading within each band. Confirm the current premium when you request a quote. Read our guide to health insurance prices in Spain by age.
What happens to the Sanitas Unico price as I get older?
Premiums move up as you cross into the next age band (65, 75, 85). However the band pricing is set, there is no individual age loading and no medical-history loading. Sanitas applies its standard annual premium adjustment in line with healthcare inflation, the same as on every Sanitas plan.
Does Sanitas Unico include dental cover?
Basic dental is included: scale and polish, simple extractions, wound treatments, dental consultations (€3 co-payment per service). For comprehensive dental cover you can add Sanitas Dental Milenium or Sanitas Dental Premium, see our dental plans.
Do I have to pay anything when I attend my Sanitas Unico appointments?
No. As a Sanitas Unico member you receive a Sanitas membership card. Each time you attend an appointment your card is swiped at reception to record your visit and any applicable co-payment. You can only present your card at Sanitas providers (full provider list in our medical directory). Co-payments are collected with your next monthly premium.
What are the co-payments on Sanitas Unico?
Sanitas Unico uses a graduated co-payment structure. Your first 6 consultations each year are free, consultations 7 to 15 cost €5 each, and from the 16th each consultation costs €10. Diagnostic tests and simple therapies cost €12 each, podiatry €3 a session, dental services €3 a visit day and emergencies €8. A course of high-frequency services, such as 10 physiotherapy sessions, has a single €20 co-payment. Co-payments are debited by Sanitas with your next monthly premium. If you would rather pay no co-payments, Sanitas Mas Salud has none and accepts new members up to age 75, subject to underwriting. Read our guide to how co-payments work.
Can existing Sanitas customers switch to Sanitas Unico?
